Instructions
- Access your online Order Receipt using one of these steps.
- Click the "Order Details" link which appears towards the bottom of your order confirmation email. OR
- Enter the order number and associated billing zipcode into the form below. Then click "Find". OR
- If you cannot find the order number, please contact our customer service department at 1-800-308-8057 x3.
- Click the "RMA Request" button and fill in the appropriate information.
- You will receive a printable RMA confirmation with packing and shipping instructions.
If your order number is NOT 6 or 7 digits long
(usually orders older than 3 years),
please use our printable form.
